Getting the penalty for an error in champion selection?

Last night while playing, my party and I encountered many queues where other players would leave during champion selection and we'd be kicked back into queue. I just thought we were unlucky so I kept on trying to find a match. The problem started when I got a dodge penalty for someone else's dodge. I ignored it the first time, and didn't think it was a big deal, until a game or two later someone in my party got the same penalty for someone else's dodge. Usually the system would say if it was someone in our party, but it just said "A player has left during champion selection" and gave us the penalty. Today, while locking in our champs someone encountered an error in locking in and the system kicked us out of selection and gave them the dodge penalty for what the system was calling an error. I'm worried that this will become an ongoing issue and that one of us will get our accounts banned. I'm also curious to see if anyone is having the same or a similar issue. I've already submitted a ticket and tweeted at riot support and I'm hoping to hear something back.
