I remember one time a guy cyber stalked me IN GAME and started sharing a bunch of stuff about me. Really creepy of the dude to do.
I got a dude who can track you irl dunno if he can do it outside our country tho. He tracked mine my girlfriends and some of our friends IP's so dont think it's impossible.
Call the police, death threats (especially of that nature) online are very illegal, and the police can get a warrant for the information of the player who threatened you, Riot will likely cooperate with them even without a warrant because Riot doesn't tolerate that behavior either.

Look at this trend that just happened. It's where someone give out another players address and got permabanned. Apparently it's his/hers first offense.
