Remake never showed up for DCed Top Laner

Our top laner never showed up and was confirmed DCed at the start of the match and end of the match (showed the DC symbol at the end game screen, and his portrait never showed up in the game loading screen) but it never gave us a remake option, forcing a loss. Any idea what went wrong? Couldn't have connected and then DCed, as his portrait stayed on "Not connected" on the loading screen.
