What do you if something serious occurs while you're in the middle of the game?

Unexpected stuff happens, which is why Riot generally doesn't punish for one off situations.
You'll lose the mmr/LP for the match of course, and you did spoil things for 4-9 other people (depending on how much the enemy team values competitive matches), but you're unlikely to get banned or otherwise punished for one off emergencies or whatever.
Of course if those 'unexpected' things occur multiple times a month, then it starts being a you issue where you're probably not taking appropriate steps to ensure you'll be free for the duration of the game you queued for, and thats when other punishments start showing up.

I think your second question is kind of odd.Of course you leave if something serious/important is happening. House burns? Go. Your spouse is in labor? Go. Someone in your house fell down the stairs? Go.
Depending on how much time you have you can write a short "accident happened, need to go" or something like that. It's everyone's own decision to report you for that and only very toxic players do that. But IF such an important thing happens to you and you get banned for it, then just take the ban because League of Legends is simple unimportant at that point.

House on fire? Leave. Kids are fighting or someone has medical emergency? Leave. You need to take your dog or pet goldfish for a walk? Plan ahead; don't start a game when you are supposed to walk them in 15 minutes.
