The issue of people not locking in bans. Solution?

For the ban-phase, if people don't lock in a ban, instead of kicking them - why don't you just move the ban to someone else, to decide that ban?

It allows more time for a person; say they're away - they may be able to get back in time to lock in their champion.

Would allow for potentially less accidental dodges to occur?

The difficulty I see in the pick/ban mechanism is the time window involved.

It can take several minutes for the ten players to sort out who they will play and who they don't want to see on the field, not because they need that long to decide, but because they are waiting for others to complete their decision-making process.

Some see this as an interactive process, a game of selection and counter-selection and for them it's a fun period of time. Others come to the table with their decisions already made, and for them it's a bore waiting for the game to start that can result in afk failures.

If someone isn't attentive enough to ban a champion, I'd rather not be stuck in a game with them for 20-40 minutes.

It doesn't exactly inspired confidence in their competence if they can't manage such a simple task.

There really needs to be no "lock" it should be the last choice and a "lock only to speed up the process. This game's pick-ban phase is already too click heavy.

Demonizing and enforcement fails to provide an entertaining game experience for the players that aren't you.
