Could someone explain how de-ranking works in Ranked?

Is there a set number of games you have to lose at 0LP to demote? From my count it looks to be 2 or 3.

Say that I lost 2 at 0 LP, but won once. Does that reset the "counter" until I reach 0 again?

Does the "counter" also count if I lose a game at 1LP?

when you are at or below 0 LP and your MMR drops too far below your rank. major divisions are more lenient that minor.
