I can't win a game, please help

I try to play passively and go in for kills when I have good vision. I try to get objectives and farm a lot. I do my best to not die. I ward effectively. I try to do all of these things but people on all my teams just die like its their job. I had a 5/14 hec recently who just started running into the opposing team 1 v 5 each and every time he revived and I see that sort of thing all the time. Tell me what I am doing wrong or please help because I just can't win games. The snowbally nature and clownfiestaness of the game makes for a very unfun experience. At this point I feel like I could do better if players would afk instead of continuously feeding the enemy team. I can't seem to stop from getting the team that just ints over and over again.
