What do you think is the most common cause of losing a match?
Title says all; What causes you and your team to lose a match? I've gathered most common flaming and accusations in the chat, and decided to make a poll out of them.

Positioning and awareness.
Specifically, not rotating properly or not prioritizing objectives properly.
Example: We are leading in gold and kills. Infernal drake spawns. Yet somehow their team is on it before ours is. This positioning and vision gives them a big advantage. We have the timer on dragon spawns. 30 seconds before, especially if you're the Jungler or Support, get your butt over there and start warding or getting in position.
The enemy team destroying your Nexus, I would wager.
I've found it to be people typing anything in the chat. When you're typing you're not playing, when you're not playing you're essentially afk and useless.
Remove "jg not ganking" and replace it with: Lack of knowledge/Experience; as this pertains to every role as well as it includes jg not ganking.
Jungler not ganking at all.
Even a mediocre gank can give a benefit by removing life or helping with the creep equilibrium being set up.
Your poll is missing AFK - D/C as my number one reason for losing most games.
The other reason would be "picking a champ that is not good enough to win" / picking a champ that does not counter the enemy pick.
But the ACTUAL #1 Reason for my lack of wins, is that my champ is not OP enough. And if the gods would buff him like they did Darius, Xayah and Sejuani...
Looking through my match history it's usually my bot laners getting crushed in lane and getting caught out late game
Tilt / not being on the same page as your team. The issue with league is there are many strats and if your team can't focus on one it really hurts (IE ace enemy team baron or inhib rush?)
where is the 'never warding' option [zombie-nunu-tears]
