I think on average since the return of dynamic champ select i have to go through about 5 dodged games. Not unaccepted, but dodged during ban/pick. I don't even want to know how much time has been wasted.

I'm guilty of accidentally dodging as well. I like ensuring I have enough time to get my rune pages/masteries right, and was last pick. I was making my mastery page when time ran out and - oh ya I forgot to lock in!
Honestly requiring people to lock in is so stupid. They say it's to decrease the amount of time it takes to make a game, but the first 40 seconds of every game you can literally DO NOTHING. Hover over the champ you "want" to play... ok? How does this decrease the amount of time it takes to make a game??

I guess its rank based. I get into games anywhere from 3-7 minutes taking into account dodges.
My anecdotal evidence on EUW contradicts this thread. So take it for what it's worth, it's just anecdotal evidence, but yesterday, first time the new champ select was available on EUW, I played 5 games as a duo, queuing for Top/Jung and my Duo for Mid/Jung, and it never took more than 20 sec to get to a lobby, and there was 2 dodges over the 5 games. Pretty great if you ask me.
I also played 4 games solo, queuing for Mid/Top, the delays were slightly longer, but still not too much, and I got Mid 4 times. I was quite surprised, after reading all the whine on this forum about Mid being impossible to get and all that. Guess it's a NA specific issue?
You don't need people to fail to accept to have a long wait time. Twenty minute plus queue times are fairly common for me lately. That's with or without having "fill" as my primary.
