This item is so fantastic on adc's and I've only just realized it.
This item offers:
-damage (lots)
-survivability
-Did I mention damage?
First of all, it allows you to stay alive longer to dish out your damage, and outsustains most skirmishes with you on top.
Now, I don't mean to rush it, far from it; but as a third/fourth item, this item is a terror when combined with 
Trust me, try building it the next time you play ADC if you don't already

Almost every ADC gets Bloodthirster. This isn't a revelation friend.

Everyone else would rather run a botrk or some other item. Blood thirster is rarely picked up these days.

Of these ADCs, the only ones who don't have a BT in their average builds are Kog'maw (scales with on-hits rather than AD so botrk is better), Jhin (doesn't get lifesteal), Varus (who builds like an AD caster), Draven (gets merc scim), Ezreal (Q procs on-hits so he goes bork), Vayne (needs bork for attack speed), and fucking Urgot, who is not an ADC.
So of the actual ADCs, only 6/17 don't get it in most games.
