There's more than that. Any enemy that gets resets can kill you because an ally screwed up and died when he/she shouldn't have. Like imagine if you have an ally fighting an enemy kat and it's a really close battle but he thinks he can win. So then you get kinda close and use a couple abilities to help out but he dies anyways. And then she just presses eqw and you die since your ally let her cooldowns reset. Then he got you killed. Yes you could've avoided it but I'd still say that's more on him if he chose to fight when he could've avoided it.
An ally could have failed to call mia or failed to interrupt that easily stoppable teleport where an enemy showed up and you didn't think they'd be able to, and again you could've avoided it, but I'd still say that's more on your ally.
There is also the idea of everyone on your team being nearby to fight in case it happens. Then an enemy Master Yi ults and charges towards you. If you just all fight him you win, but all your allies choose to just run away, so then he just kills you one by one and no one of you can stop him. This is totally the team's fault since you needed that positioning you had in order to win the fight, your team just failed to follow up on it correctly.
And of course, we need that simple old 10-0 enemy top laner just teleporting to your lane and then they can just dive you so easily since he's so much stronger than you are. And though technically this is your fault, there's usually not much you can do about this.
