What separates a Good support vs a Great support

So what makes a good Supp great?
A good support will have items with actives and occasionally use them right. A great support will have items with actives and know when and how to use them right, Always!!!!
Seriously, how do people do it all the time, consistently!!! My word, I just had a game. and i had to use all my knowledge as a Support, No, A league of legends player to use them all the time, correctly.
When to use Mikael's to stop the Ahri charm from killing a high priority target
When to use frost queens at the right time during a team fight (look for possible flanks or save it to chase down stragglers)
To try to negate the Brand Ult damage
Use this to stop Jinx/Xin from going ham on us
Plus i still have to try and used my abilities.
- Heal the carry
- Bubble the high priority target
- Should i ult to engage, or dis-engage
I only have so many fingers.
