For the masteries, not sure what "the champ permanent of the same champ" means...

I don't know why everyone forgot this. The lootboxes have a small chance of giving you a permanent unlock instead of a shard: it looks like a shard, but you don't have to spend any dust to own the champion. If you already own the champion and get the permanent unlock, the unlock just sits in the hextech crafting shard area.

So if i have a champion that i buyed and i have it on lv 5 and have 2 mastery 6 tokens i just combine the tokens and i got mastery lv 6 on champion?

No. You do have to own the champion, of course, but having the champion shard or champion shard permanent is a separate thing.

Picture showing Shaco Permanent, and a comparison of a permanent to a shard: http://i.imgur.com/D1biRFv.png?noredirect You need 2 Mastery 6 tokens and either 500 blue essence, the champion's shard, or the Champion Permanent to combine with the tokens. Unless you get lucky and happen to get the shard for your champion, in most cases it will be more efficient to go for the essence.

I've never heard of anyone getting a permanent from a chest outside of when crafting was only on the PBE.

Permanents that you get from rerolling do sit in your inventory before you claim them, and if you don't want to claim them you can instead disenchant them for essence.

Actually, now that you mention it, it seems they did nix that idea before it hit launch. It's not in the support article. Oh well.
