TL:DR post, Short Answer I agree.
Not sure if its the wrong board or not but I agree this is a major problem, there are many examples I could give where players are just flat out toxic in champion select because of non communication going back to this post and response posted by Riot many months ago it is a serious issue. http://forums.na.leagueoflegends.com/board/showthread.php?p=37793026
Based off Riot's post (Refusing to talk in champion select) is toxic behavior and if reported is punish worthy, over half of my games which had toxic behavior are due to players who come into champion select and do dumb things, don't talk, instantly just ban champions despite what players on the team say to do, or make toxic comments when someone says they rather not play (X) role or champion at (X) location you get called a n00b, told to uninstall or don't play ranked etc.
There are a many number of examples I could give of toxic behavior the last few days, but in champion select is where a lot of it seems to be players just come into the game, they don't listen to what others say, like in like 2 of my games earlier I had players not listen when I said bot lane because immediately players called top/mid, they completely threw the game off and started harassment in chat, in another game I had an ADC who refused to follow up at all with trades they would just troll and I was the one dealing all the damage in the bot lane but they ended up getting kills earlier because the adc was the most useless person ever, the game before that was a loss, but the ADC actually did a great job and it wasn't their fault and I was playing the same way. I tell them in chat the game play has to change, the other player on the team starts yelling toxic comments in chat, and makes Real Life threats.
There are a number of things that are toxic the most toxic is when players get verbally abusive and start to threaten others with reports this generally starts chat toxicity,and really pisses me off when the players being toxic and filing false reports are the ones who did not listen in champion select and trolled the game, but there are other things that cause game throws very easily.
(Here are a few things I can think of)
. Players who do not communicate in champion select.
. Players who don't listen when someone says pref not (X) Role (Then players get angry when a player does bad.)
. Players who don't pay attention to calls in the game, example Barron, Dragon.
. Jungler who (Fail Gank) and keep doing it this is not as common but sometimes you get a toxic player who refuses to back off and causes you to lose lane especially when they keep pinging and you don't need a gank because you are winning 1v1 early game between level 1-7 and can easily get ahead but your jungler intentionally throws off the lane.
The best time I personally go into lane ganks is when an enemy is over extended, me or my laner have slow, or CC ability, snare etc, or the enemy is for sure going to be able to be ganked successfully, or the enemy jungler enters a lane then it becomes a 2v2.
. The best way I have found to keep a game from getting messed up, is (Let your jungler make their own decisions) if the jungler wants to invade, then they call an invade for example, other things like Dragon, or Baron, your JG generally makes the call to take it sometimes players don't, but sometimes players get toxic in chat when something goes wrong after the jungler did not make the call to do an invade, or dragon or something. (In general if something goes wrong in the jungle because someone else made the call to do something other than the jungler then its your fault.)
"Two of the most common things are making the decision between taking a tower and taking a dragon, If you have just won a team fight and can take a tower + a dragon take both, otherwise take towers first usually then a dragon, or wait for an advantage before taking a dragon unless you want to force a fight at the dragon. The other common mistake I see is taking Baron, players like to do this when all 5 players on the enemy team up this is generally a bad idea, unless under certain situations usually in the lower ranks like Bronze where your team is really fed, or when the enemy has to defend base or when you have an advantage over your team and can win a fight should the enemy engage. (Always ward the area of baron before trying to take it." Another idea is if your jungler can solo dragon even better as long as the area is warded, or your team is in a 4v5 keeping pressure.
Another example is players who demand ganks because they lose the lane mostly due to be unskilled, and you know the gank will go bad but they do not listen and get toxic this way.
. Players who let their champions roam and do absolutely nothing about it, a good example is one of my games with toxic players, we won bot I got my ADC like 3 kills at least we did great together, then all of a sudden here comes the mid lane, (No Missing call) but worst of all our mid lane player did absolutely nothing about it.
. Summoner Spells do not mean that a player is playing (X) role, sometimes a top lane player may choose to take exhaust for example.
Players who do not think about Team Fights, instead they are most interested in winning a 1v1 lane rather than actual Team Play, well I will tell you why games are lost is because of players who absolutely play like this.
Let me give a few examples of toxicity that causes games to be lost from some of my recent experience related to this.
In champion select, we get a player who yells MID, and has the whole Solo carry attitude, I will get fed I will carry you baddies, okay game starts, they get fed, but at the cost of letting their champion roam bot and ruin our lane phase as well as take our turret and doing nothing about it sure this player went 15-5 mid lane and did great, however here was the problem everyone but me as a player failed to see, our team had (No CC) our top laner meanwhile went Kassadin into a Lissindra, and went 20-10, but that didn't matter the main reason for the loss. (Free Roams from other lanes) two players on our team did not use their brains and were toxic, now the enemy team compared to ours won because.
. They had Crowd Control, They had Stun, and Lissandra an ultimate that takes out an entire champion on our team for 1.5 Seconds, So who do you think is going to win when two of us get way behind because our mid laner has the toxicity and attitude of I will carry while letting their champion roam Lissandra will simply use her ultimate on one of our fed Team Members in a group fight, or ult herself in the middle of all of us, our support has a Stun, and CC and will simply stun the next person doing the most damage, while the other two + adc deal the damage easily taking out everyone on the team due to no CC and thinking or communication from these two players.
In general just because a champion is listed here http://championselect.net/ as a counter doesn't mean you are going to win team fights especially when you do not have hard (CC) think for example Lissandra vs Kassadin 1v1 sure you might do great, but during 5v5 fights forced fights which happens quite often there are better choices especially in lower elo like a jungler such as
for freeze +
for lane against Lissandra, and why would this example be better, well you got a Freeze for 1.75 seconds, and a brand ultimate that bounces between enemies doing massive damage to more than one enemy rather than a kassadin who is only focus on a single target. There is a huge difference between 1v1's all game long and if you are expecting to fight 1v1's all game long then League isn't for you because that isn't going to happen eventually you are going to have to Split Push, or Fight as a group and be able to win especially when the other team forces a fight.
On the other hand games where players immediately lock mid, and just lock things like
and a team who doesn't communicate at all while the game was a win only because the enemy was unskilled and my team was toxic to the enemy team as well, the point is rather than having to have toxicity, and players who trolled in champion select, we could have had a way better team composition built around Yasuo, but instead no communication, and 3 toxic players on the same team starting with the support who did not listen in champion select when things were selected, and two other players following toxicity, I am talking about something like.

Riot should really start to enforce communication in champion select rather than players going afk, or trolling, and there is absolutely no way to dodge a second game in the same day without having to wait 30 Real Life minutes and lose LP on top of the 30 minute dodge penalty when someone chooses to troll or be toxic there is absolutely no way to report it unless you play the game with them, and I am not talking about things like breaking a Meta, Riot doesn't enforce a Meta, or any type of game play tactics, I am fine with that, but I am rather talking about the people who don't talk in champion select, and I would hope that the new Tribunal will have the Champion Select logs to better reveal those who communicate vs those who don't when players are reported because god knows how many times I have personally been reported by toxic players just today alone, although the most I have gotten is 23 chat restriction after being reported numerous times by players who do harassment in chat all the time and just won't shut up which these restrictions don't really impact my game-play although it does rather piss me off when the players who are reporting others which get them chat restrictions whoever the reported person maybe are the toxic players themselves. http://i.imgur.com/pUMjMww.jpg For example without names.
