Tribunal Punishments

Here's what i know, please correct me if i'm wrong.

1. The punishment phases for feeding/being toxic is: Ranked restrictions/chat restriction > 14 day suspension > Perma-ban.

2. The punishment for cheating/hacking is an instant perma-ban.

3. The punishment for leaving a game(being detected by Leaverbuster, not just afking and having the popup show up saying you're afk) is: After being detected twice the steps of punishments you receive are: 5 low priority queues which lasts 5 minutes for 2 leaves > 5 low priority queues which lasts 10 minutes for 3 leaves > 5 low priority queues which last 30 minutes for 4 leaves > then the last is 1 hour for 5 leaves?

4. How many games, after you've already been punished, does an infraction last until it expires and tells you that you've been a good boy? (until you can start receiving rewards again) or is it time-based and not by the games?

This is all from my experience with myself and from people i know that have had these happen to them so i might be wrong. Does anyone know if there's a place or discussion where it tells all about the tribunal?
