literally 4 or 5 people play him top lane on regular bases. not even mains. his MAIN MAINS
And a 0.15% playrate and only 366 games, as well as almost no significant data on what contributed to it. That means almost nothing.
Sample size on champion.gg is 366 games. Playerbase average games is 125, and the sample was taken from Platinum+. It's not a surprise at all, nor is it that high given this info.
If you, or I, or anyone reading this tried Karthus top, that lane will be an all-you-can-eat buffet.
Too many assassins and burst mages mid. In top lane, at least he can farm safely and scale up.
inb4 riot guts him for it (even though it's more of an outlier case) while continuing to compensate Zed for every change they make to him

Like the guy above said. Look into the data more before going all crazy.
Gets to farm all game in top lane and then suddenly ults the enemy team for half of their HP. Turns out that's a pretty good strategy for winning games.
On a related note, Urgot top at 54%.

He became more powerful in the recent mage update with more cdr items available.
Yeah, but this has fewer than 300 games total.
Karthus mains can pull it off, and they are the only ones who are doing it well.
The reason it works for them is that, in a good matchup, Karthus will get 20+ minutes of uninterrupted farming, which is pretty much exactly what he wants.
just had a game where enemy top karthus fed his ass off.

Karthus
top and mid have had crazy high win rates in Diamond for a while now. Often times around 60%.
So as long as he doesn't build tank stats I'm fine with it
