Need advice on winning my ranked matches!

So ive been having troubles lately just winning. Im a Plat 3 player and an urgot top main.

So my current situation is that i always have that 1 teammate who tilts then runs it down their lane. Im usually able to get them back into the game, trying that is, but usually its too late. Im able to carry matches out into the late game, but usually its just too late. I do acknowledge that im not great at the game. I do sometimes feed and tilts (usually my fault for playing ranked matches when tired.)

I'm just hard stuck Plat 3, and i keep winning then losing and vice versa. I wonder if there is any advice i can be given. Im having trouble this meta bc i only can carry for so long until the enemy adc out scales me. This is only my 2nd season (I let my friends play 2 seasons ago since i was busy playing csgo). I really haven't gotten any constructive criticism and i think im at the point to where thats what i need to start winning my matches along with actual good advice.
