True Damage Counter
With all the %health damage and true damage. How would the addition of a counter item be?
Maybe Passive: Reduce all incoming True damage by 20%. stick that on warmogs or something.

And there should be a acounter item to this item that reduces this reduction by 30%.
Regular true damage is already countered by stacking hp.
%hp true damage is a lot trickier, and depends more on the champion you're against. AS slows will reduce Vayne's damage, good positioning will reduce Fiora's damage and kiting reduces Garen's damage (though if you're a frontliner, you can't really run away from fights).

if darius or garen... you have no hope... just run or flash away if you are on low health.... if vayne... get fh... ro sucks now if ahri... stay away from her skillshot if ire.,, fight her like man... hydra, botkr or bt will save you
The whole point of true damage / %max hp true damage is that you aren't supposed to be able to buy raw stats against them. You have to beat them with your actually gameplay.

You can't itemize to damage invulnerable targets, send projectiles through projectile-blocking shields, gain 100% accuracy vs. 100% evasion/blinds, apply CC to enemies that are immune to CC, or impose a mana bar on manaless champions.
Some champions just have strengths you have to play around rather than trying to out-muscle. That's really a big part of strategy: exploiting the weaknesses of an opponent with your strengths. Not so much running into their strengths headlong and trying to counter-them.
As much as it sucks as a tanky DPS or tank when you run into someone with defensive-busting capabilities, nobody else really has a persistent defensive mechanic that's always at 100% either, so when you run into %HP true damage: use them problem-solving skills to come up with a solution that isn't simply outlasting an opponent that hit you multiple times because you bought defense.
Make an item that gives a regenerating shield instead of hp, armor, or mr. This shield can't be used against you for %hp truedamage
