The problem (in my opinion) is that against some of the most prominent cases, it- or rather, it's followup item Mortal Reminder item 3033- just doesn't cut it. 40% healing reduction is good, but it doesn't matter when the enemy already has a massive health pool on top of it. Mundo, Swain, Garen, Darius... They build huge piles of health and tank stats, and you ultimately end up in a "damned if you do, damned if you don't" scenario; Lord Dominik's Regards item 3036? They can still heal off from your damage. Mortal Reminder item 3033? They still have huge amounts of health you have to tear through.

The items are unfortunately flawed in that they're meant to deal with enemies who build either sustain or health with tank stats, when many often-played champions just build all three. And since there aren't any viable alternatives (BotRK was nerfed, the only other GW item is Morellonomicon) you're forced into an inefficient build path of getting both.

who cares about the follow up item? item 3123 on it's own is important to have...

Yeah, actually building Dominik+just an executioner without upgrading it is a viable option until you reach the late game where you need that last slot.

Mundo may be tricky to deal with, granted. But if you build Mortal Reminder against Garen or Darius, you're doing it wrong. Garen regen is out of combat, so Mortal Reminder is no use at all. Darius heals only with his Q, and it's not worth going Mortal Reminder over Dominik just for that.

On the other hand, Swain has a lot of HP for a mage, but he doesn't have a huge pile of health to tear through... So a Mortal Reminder should work pretty well. Same against a Aatrox, Fiddlestick, Vladimir, and other drain tanks. They usually build more offensively than true tanks, and don't have so many HP.

The problem is when you have both a Garen top and an Aatrox jungle, and both are fed, in the same game, because there you need one item for each. But saying you need both items against any single tank, it's not true.
