Can we change the fact it applies no matter what % ur at and from any source of physical damage?

its an exelent counter to yas atm
They also changed Grievous Wounds to not affect healing that comes from allies, so actually got a huge buff out of it.
Who will really suffer are 
etc, aka the ones who rely a lot on their self healing during fights...
