Issue with surrender voting

Ok so this just happened to me. My team was doing a surrender vote, and I had my shop open to buy a Rylai's . When I clicked buy, it said I voted yes to surrender and we surrendered. Has this happened to anyone else?

I just wish when I answered personally that it would disappear from my screen. I hate how it just sits there while I'm trying to play league and can't see a valuable portion of my screen. Its small yes, but it does impact my vision.
