Advice on getting out Silver 5?

Retry next season and hope in luck during placements.
If you aren't dropping or rising, it means you belong in S5. If you can acknowledge this fact, then you can start to work on improving. Improve on all the little things, last hitting, rotating around the map, prioritizing objectives (baron, dragon, towers, inhibitors, nexus) over kills. Outside of improving your play, there isn't much you can do outside of yourself. In general, its not a good idea to point out a player who is doing bad because they get defensive and can decide to throw a tantrum and throw the game completely despite it being winnable still just because they are having a bad game. The best thing you can do in these situations is just stay quiet and try to make something out of their bad play. If you have the knowledge that someone is playing bad, it means that you have the knowledge to get better (hopefully), so that all you need to do is execute better. Once you improve yourself, you should be able to rise in ranked and do go up in tiers/divisions. Also you can try once S5 hits, but know that if you get put back in Silver V, then it most likely means you BELONG there, so you will have to work on improving yourself from then on or just in general if you want to go higher.

There's no magic "elo heaven" where your team never does dumb shit. You have to learn to minimize your own mistakes and to play along with what your team is able to do. You need to be able to watch for strategic objectives that are easily available (such as a "free" tower, "free" dragon, etc), and ping them for your team.
A silver 1 player placed into silver V will always move back up to silver 1, and they have the same amount of "dumb shit" happening in their teams. If you're playing a bunch of games and still staying in silver V, it means your level of play really is silver V.
I recommend concentrating on the basics. Whenever you have an empty slot and 75g, buy a ward. Check your wards placed at the end of every game and try to get it above 15 per game every time. You might even try building a sightstone as jungle/mid/top. It's totally feasible and very worthwhile. With more vision on the map, your team will do less "dumb shit" and start making better choices.
Just keep trying to find ways to minimize your own mistakes. Review your games using the op.gg replay system until Rito finally enables their own replay system (Soon(tm)). Try to find one main thing that you can practice in every game. Try to die less in lane, etc.
Also, go check out the series of videos "Leaguecraft101" and watch the basic videos and the ones appropriate to your main role. If you practice all the things mentioned in there, it should get you to Gold.

I am having similar issues, I was in Silver 3 and rising but then I suddenly hit a HUGE losing streak, mainly due to some lane or another feeding, an AFK or (my latest incident) terrible lobby communication resulting in a duo top, my playing a champion I wasn't very proficient at, & a Jungle Lee Sin making my game a living hell since every bloody time I charmed that Ziggs (I was on Ahri), he would be there to Sonic Wave me in the face and get Ziggs another kill :( (Yes i had wards out but due to our lack of jungle Lee was using both jungles freely meaning a lot of the time he came from behind to kill me where my wards couldn't see). In the end I was so behind and my team was so scared to team fight that I would die every time I attempted to go onto their carries from a storm of CC that I would get slammed with. Not that it even mattered since I was so behind my full burst rotation with DFG didn't do enough to kill anyway T-T.
Stuff like that has been happening for my last 8 ranked games, I was in Silver 3 promos then I just started to lose and lose and lose. The one good game I had I got to full build insane damage mode Syndra, but we couldn't do anything because their AP Cho would lock us up then Riven or Twitch would shred my team, I could always remove Twitch or Riven with my burst combo but the other would live to wreak havoc, and AP Cho was dealing like 600+ damage per spell with over 3k health :(
It sucks but I'm just going to keep going until I hit Bronze or Gold (hopefully Gold)
The best thing for you to do is pick a role and play that role as much as you can. For me i picked support and i was able to learn match ups and improve my play faster. Also being positive and looking at all the ways you can improve, even if it means warding more, or buying pinks more often. Counter picking can help, but it really helps when you can play the champ well too. I recommend to keep playing ranked and learn from your mistakes now so you can get better placements next season.
Try to communicate with your team.
I had an amazing game last night because I noticed that the enemy Blue team's first 3 picks had large AOE effects that we probably never would want to team fight against (Xerath, Gnar, and Karma), so I told the rest of my team to choose heavy split pushers and good duelists so that we never have to deal with the enemy team's AOE team fights. In the end, my team's Fiora snowballed hard and I roamed plenty with Master Yi, which resulted in multiple towers falling extremely quickly and extremely powerful dragon control thanks to our constant split push rotations. In other words, that communication pre-game, in my opinion, ended doing a whole lot towards winning the game for us.
There are also tips that are very specific to certain roles. Mind me asking what roles you prefer to play?
Kinda late in the season tbh, lots trolls with the sole intent of not letting people hit the next ranking tier for better rewards
