How do I get better?

Hello, I am a silver player looking to get better at the game, and I'm looking for tips to get better.

One video I watched stated that the main difference between challenger and bronze is the amount of mistakes they make every minute, and the reason many can't climb is that they don't know some of their decisions are mistakes. Thus, I am asking people to tell me what I am doing wrong, so I can improve at the game.

The link is a google drive to a replay of me playing Yasuo. I also want to know how I could have won this game, preferably after the 30 minute mark (game is 41 minutes). I could link more replays if anyone would want it.

This isn't a post talking to nerf champs or rage, just looking for constructive criticism.

My mains: Top -- secondary role: Garen, Darius, Gnar, Shen, Nasus Mid --Main role: Yasuo, Talon, Ekko, Veigar Jg -- Jarvan Bot --Jhin, Jinx Support -- Leona, Blitz, Soraka

Note for replay: Singed wouldn't listen to us. Xin tried to lead the team, but liked to run off without us and fight (could be my mistake)
