5v5 ranked Team game

How do you create a team game now with the changes? No option appears for ranked teams and even though riot put in the patch notes that you can 5 man premade ranked that isn't the case because all have to be within 1 division of each other to rank. Any suggestions __ From the patch notes: Ranked teams will still be available for players who want to form a team of any skill level, as teams are judged on the skill of the static group rather than the sum total of the individuals.
