Hotkeys randomly don't work.

I'm having a problem where my abilities won't go off, even if I press the button multiple times. It usually happens when I get to button mashing. In a teamfight as Yasuo, I hit a tornado on their whole team, started mashing R, and it never went off (and I wasn't cc'd, this is just the latest example.) Same game, trying to escape, I started mashing F to flash and my flash didn't go off till about the 4th or 5th time. It happens to me randomly, and on all the champions I play and I can't figure out what is wrong or how to fix it.
