Soraka is the only champion that has done health-costs really well.

Soraka's health-costs actually makes you play around it in a meaningful way. Zac and Olaf does it okay to some extent, but not nearly as well as Soraka.

What makes it work so well is that, if she so choose, she is free to eat through her health rapidly by spamming her heal, which makes her maximum output put her at much higher risk. This is combined with her source of regen, which is a skillshot. Her sustain is therefore by no means guaranteed, which allow some interesting choices for her between how much health she decides to spend depending on how quickly she think she can manage to regain her health.

Regen from auto-attacks are problematic because it's too automatic, literally. This makes it a stat-check and low risk, which doesn't really change your play, high health or low. It's just a snowball-mechanic at this point, rather than something that defines a champions playstyle. Therefore, on someone like Aatrox it will only add a speck of nuance, at best.

Similar issues are found with "sustain-tanks" without health costs. An Akali that sustains through retaliating damage doesn't play around it per se, it just puts her even further ahead the more fed she is. Same goes for Warwick.

Mundo is also really bad in this regard, as he doesn't even have to auto-attack. He's effectively just a ball of tank rather than someone trying to balance his loss- and regeneration of health. He's not a regen tank, because the exact same result would be achieved with just a lot of health, especially because of how innately tanky he is. It doesn't help that his health-costs are so small.

Bottom line, the cost must be meaningful and much higher than the regenerating effect if not managed really well. Auto-attack regen or flat out % of damage are bad mechanics for a sustain-tank because it's too predictable regen.

I hope those champions that are supposed to be about health-costs are reworked with Soraka's design as a guide.

Tbh, in 5.16, get deadman's plate, spirit visage, and warmongs and you are god.

I get a spirit visage for the passive but I don't like stacking health on her. Her heal costs a flat 10% HP no matter how much health you have, her q at level 5 heals her for 65 + .4 AP + 1% per 1% missing health for each champion hit. When you stack HP you get a heal that cost more and a q that seems to heal less.

PBE change for Warmog = Passive Changed: Grants Warmog's Heart if you have above 3000 life. Warmog's Heart restores 3% of your maximum life per second when you haven't taken damage in 8 seconds.

It's really good on Soraka in my opinion.

But that means your heal will use over 300 health. Her base health is 5291855, so you need another health item other than Warmog, which will bring your health up to 13292655. (Of course you can't get Warmog's at level 1, but I'm just using that scale to show how much health you'd have mid-game. Spirit visage add 400HP, which brings her health to 1729~3055. She barely reaches 3K health by having both of those items and level 18. She'll need another health item to actually have Warmog's passive proc before she's level 18.

She also needs item 3069 and boots, which leaves 1 slot for AP. Let's say she gets Rabadon's, which gives the most AP. 120*(1.35)=162. With this much AP, her heal will (at max rank) heal by 240+162*0.6=337.2. That's nothing.

It might be worth it if you run HP masteries and runes, but overall I think AP Soraka is much better as support.
