lol when you are in champ select see the enemy pick their champ and you know the perfect counter and picked the wrong champ instead by mistake
i saw a teemo i was gonna pick galio and some how i locked yorick

sounds like a personal issue to me.
i don't think i have ever done this... but just a few day ago i was last pick and went to the rest room.. and while sitting i was like.. "oh crap i didn't take smite" and i couldn't run back to my computer before it loaded into the loading screen. XD
