What the Hell causes all these random bugs?

Its because riot's code is messy. Its a ongoing joke that riot's code is "spaghetti code" which means its all tangled up, and basically if you do something with one part it may in fact cause an error somewhere else. It's a problem though, because when they made the game back in 2008 or 2009 or whenever they probably didn't expect to get this big, and back then the code was very rough for that reason. But then they built on top of that code without fixing it. And they still continue to build on top of that code. So it would take a huge huuuge rewrite of the entire code to fix.

Because this issue wasn't tied to a specific champion, I actually suspect that this specific bug was introduced a while ago and it took some time for someone to notice the glitchy behavior and find a way to exploit it.

It's very hard to test every nook and cranny of the codebase of a game the size of League. It's pretty much impossible when it's constantly undergoing changes.
