New Team Rewards (in patch 4.9)

I figured asking here would probably elicit more of a response than in the seething mass of comments on the actual update 4.9 post.

Are the 2014 team reward ward skins unlocked for each individual player on that team to use whenever they want (e.g. solo/duo queue), or are they restricted to instances where you're playing with that specific team? What happens if you earn the reward but the team breaks up for some reason?
