Tiamat passive didn't work

I played as Jarvan yesterday, and bought Tiamat. Once during the game when the active on Tiamat was on cooldown, my basics didn't cleave (didn't deal AoE damage). Haven't happened before, and haven't happened again, so it's probably very rare, but the bug do exist.
