auto decline during queue

This happens rarely but sometimes I queue up and the decline accept option never pops up and the game says I declined the match even though I didn't do anything. I usually just queue again but this time it happened 3 times straight in a row and now the client says i could get penalty for declining too many games when I did not even decline or get the accept/decline option. Please help riot, I do not want to get a low priority queue when it's not even my fault!
