Koi Nami Element (Purple Chorma Pack) basic attacks deform Nami's avatar

When I'm playing Nami, occasionally while using basic attacks her champion's avatar deforms. It looks as if she looses her arms entirely, her staff goes through her chest cavity, and the end of her tail end tends and connects to her hips (eliminating the rest of her lower body entirely).

This seems to only happen with one or two of her attack animations, but it is a consistent problem.

(Note: this is a problem I was experiencing in 5.16. I haven't tested it out in the 5.17 patch yet)
