[Gameplay] Vel'Koz ult not firing, then going on cooldown

I just encountered a strange bug on Vel'Koz. I was playing hexakill, and just as a fight broke out I hit R to start my ultimate. The sound effect played for about two seconds, and the ability went on cooldown, but no laser came out. I think Vel'Koz himself also struck a strange pose as if bugged out?

I am 100% sure that I was not silenced or stunned in any way.

I can't really give you the steps to reproduce this, because I don't know why it happened. It only happened once and I'm not sure why.

Do you mind posting the enemy champion line-up? Most likely you were affected by a subtle channel-disrupting skill and didn't notice.

it happened to me in hexakill! I thought maybe I interuppted it somehow, but I could have sworn the enemy was not attacking me , nor was there anything around me or hitting me. It just didn't work and went on almost insta cooldown. Only happened that once though,

I can confirm, happened to me several times in a game I've played with him.
