Critical Visual Bug That Makes the Game Unplayable (unfixed for many seasons)

For inexplicable reasons, the camera will sometimes lock (my middle mouse button seems to trigger this more than other keys but it seems random). This makes team fighting and playing the game properly impossible for anyone that doesn't naturally play like this. Pressing Y (or whatever is default for camera lock), clicking the icon and all manner of other tricks do NOT resolve this. It's a game breaking bug that I've lost countless games from. The only thing I can do to fix it is to quit the game and reconnect. In any close game, this is a death sentence. This needs to be fixed ASAP. It's extremely serious. I have lost more games from this than anything else. At the very least have some option to just altogether disable locking camera so this doesn't occur. I am literally never going to lock the camera out of my own volition in game.
