[CLIENT] Launcher Glitch

I queued up with a few friends trying to play a normals game, and it glitched out and, although everyone accepted, it glitched out and assumed someone didn't accept. It put me into the pre-game queue, while my friends are in champ select, and then I am now on probation. Can someone explain to me what this was? Was it a glitch in the launcher? Or something else?

Sadly, I haven't been able to reproduce the bug, and haven't got any sort of image or video of said bug, If it happens again, I'll put one up and make sure to remember how I did it. I think I might have an idea, but it's only a hunch. Much appreciated if someone who is having this issue could help too.
