"an error occurred locking in champ" during ban

I banned a champ and I locked it in but then the countdown timer was still going and I couldn't do anything. Couldn't choose anyone different, or click ban. I got a dodge and an error message came up saying an error occurred lock in the champ.

I don't understand what happened. Has this happened to anyone else?
