It doesn't work with Mordekaiser's Q's either. It's how it's designed cause it would be Bullshit otherwise
Working as intended, Riot intentionally made it say, "next three basic attacks." Since the Guinsoo's just applies on-hit effects twice, it doesn't count as two basic attacks. Riot even specified that this was the case in the patch notes a while back
