Illaoi ult bug?

Anyone experienced this recently? While using Illaoi I ult with 2-3 enemy champions in my ult area when it goes off, but no tentacles spawn. According to the skill description, one tentacle is supposed to spawn per enemy champion hit by the ult. This happened more than once, but it doesn't happen every time. Anyone experienced this?
