Zed ult indicator not working right.

I played a game earlier today as xayah, and the enemy team had a Zed. When Zed ults you and your health gets low enough a symbol appears above your head showing you are going to die. One of the times he killed me with his ult the symbol didn't appear. I had no idea it was going to kill me otherwise I would have used heal. The second time he ulted me it appeared and I mentioned in all chat that at least it had appeared that time and the zed said that it was always over my head. Every time I died to it since the first time it appeared.

i heard that he was hotfixed today or something for some bug. idk i had one last game.

This has been a thing for a long time now, especially with shields or ignite, i main zed with around 300k on two accounts and this bug type thing has been around for quite a while, and happens quite frequently. It mostly comes to be an issue with dot effects and shields, since it determines whether or not a shuriken will be displayed depending on current health vs death mark dmg. For example if zed had elder and/or red buff and death marked you, chunked you, and you didn't have a shuriken for most of the death mark delay then the burn of dot might bring you to the point you'd die just as the mark pops, killing you without a shuriken displaying. Same with ignite, and a shield that would negate death mark will also remove the shuriken or not display it to begin with, if the jannna or whatever cast the shield at the wrong time on a death marked target and the shield runs out just before the mark damage is applied then there would be no shuriken, but the second after the shield runs out you'd still die. It'd be cool if riot fixed death mark to take dot effects into account but i doubt it will happen.

When it killed the first time was it with a thunderlord's proc? It won't show up if the ult procs a thunderlord's since it's the keystone that actually deals the finishing blow.
