Skills not being Leveled up After Clicking Them

Has anyone had this glitch where you level up one of your skills, only to try to cast it later and see that it never leveled up? I've had this glitch in maybe 1 every 20 games I notice it ever since I started playing 2-3 years ago. I've reinstalled the client 4-5 times since I first installed LoL.

And no it is not my mouse - used 3 different mouses since I started that all work fine and still get this problem randomly.
