Item Sets occasionally rollback to an old set

I've had this happen a couple of times now on different champions. Basically, I had an old item set saved, I changed it, played a few games and the changes went through as they should have, and then all of a sudden I'll get into a game and notice the item set is the old one I used in the past. After coming out of the game and checking the Item Sets tab, it is in fact rolled back to the old item set and I have to go recreate/save the new one again.
