Gameplay-Rammus permenently stuck in W form

This has happened to me for a while now, basically I'm permenently stuck in the visual form of W (without the actual effect, it doesnt effect anything but visuals, but my friends can see it too in game) just without the W bubble. It looks like im crawling everywhere. I would screenshot but I don't really know how to reproduce it.

My theory: It happens when I die by like towerdiving early or something (usually occurs first 20 min so maybe low lvl w?) and I die right as I'm transitioning back from W into normal.

Please fix! It's really annoying, I like waddling around.
