People always get into the notion that since they died once, they won't die again. Then you see them engaging when they shouldn't because thy have that "I can beat them this time" mentality, and that is when Feeding occurs.

I think people should unlearn, "I think I can" attitude after they give up 2 kills. Now these kills are just from the person who died from an engage. Not from the other adc/support going in.

depends on why u died, coudlve been something stupid ur jungler did, support, adc...death isn't really that big of an issue as u think it is. if im top and im first to die due to gank im not gonna give up my pressure of top, I don't mind jugnelr coming all day top abandoning his objectives to secure dragon etc. I can die as many time I want top if my goal is to pressure 24/7

well, if you were able to buy an item and they stayed pushing tower you can almost always get a kill as soon as you come into lane.
