You post a "help me" plea between 2 and 3 in the morning (Central time) and complain when nobody responds to you right away? Please...
Now to be constructive: your CS numbers are not Gold quality. As a mid laner and an ADC, you never once see a 200+ CS game (a mark you should be hitting by the 30 minute, at the very least). Even as a jungler, you should have way more than 100 CS in a 40 minute game. That is a LOT of lost gold. That is a LOT of items you should have. At 40 minutes in, you should have at least 3, if not 4 completed items and be finishing off your final build. It looks like in a lot of your games, instead of focusing on objectives and getting a quality gold per minute, you are turning your games into Team Deathmatch and looking for kills. Kills are all well and good, but a couple waves of minions get you just as much gold. If you are making less than 300 gold per minute in a solo lane/carry or as a jungler, you need to re-evaluate your farming.
Also, in my experience, I have seen Hecarim jungle does not have nearly the kind of impact on a game as Hecarim top lane with teleport. There is a predictability to ganking paths that you have available to you. Teleport lets you come from all kinds of places they would never expect, especially with the Homeguards enchant. I noticed that you build the warrior enchant, into Trinity Force, but that leaves you very squishy and vulnerable to being focused down once you show yourself in lane. Unlike top lane, where you are gaining continuous solo lane experience and can build a level lead over other lanes, a jungler has to farm really hard to keep up with levels of just the duo lane. That means that most of your ganks will be against champions that have stronger levels than you in the mid and top lane. That also puts you at a disadvantage in team fights later, as Hecarim is a skirmishing champion and any CC or focus from the opposing team will tear through your lack of tanky items (since your focus is on building movement speed and damaging items early on).
You mentioned that your KDR doesn't normally look like this until later in the game, but when you die 6+ times, it doesn't matter when it happens. Just that it does. It becomes even more important that you don't die later in game because that means that your opponents are getting 40+ seconds that they don't have to worry about you being on the map. That is a lot of objective control and ward placement that they are getting, especially if you are the jungler. That is extra dragons and those late game barons that crush you at the end of the game. Reducing your deaths means that you're reducing the times when you are taking unnecessary risks (wandering through the under-warded jungle, jumping in on a 2v1, chasing a support into the fog of war, etc.) It's also denying your enemy that kill gold, which means you are making them work harder to win. The more times that you or your team dies, the greater your chance of success slips away.
Those are just a couple of ideas on how to improve. I wish you luck in your climb.
