Hey Diamond Players, My Team Needs Some Help On Warding

I'm a mid/high gold player on a mainly plat team. We are having difficulties with wards. Not completely sure if it's just not being consistent with warding or just the quality of the warding itself. In the past three games we have been split pushed all the way to inhib by the adc alone. We don't notice it until they are already there, partially due to other lanes pushing harder with no poke or anything to compensate for the other push. I guess what we need help on is how to get through the early and mid game with complete ward control easy, obviously winning lane is the main part, but where completely is the best warding? Also not only warding how do we take wards out effectively at a higher rate to help us have the advantage? I know communication is a big factor into all of this but anything would help.
