i have studied your match history and i have noticed that the enemy is killing your nexus before you are killing theirs.. I will do more research, but i am 75% sure that is the reason you are losing games.
To get out of bronze all you need to do is set a cs goal for yourself, avoid dying pointlessly, ward, and dont chase into unwarded jungles. These are the things that had me in bronze for like a year before getting out

But before I do that I have a question. Is this your first season in ranked? the new op.gg layout is confusing to me so I don't know where to check if you played it last season x.x
If it's your first season then it's not too bad. I spent most of last season in b5/b4 before getting back to b1. Just hit level 30 so I jumped right into ranked. I was an idiot x.x You gotta practice more. Also most of your top champs are all late game. This is bronze buddy. A game can easily end by 20. If you can't reach the late game then you will never hit the power you champs need. try to learn more mid game and early game champions.
If it isn't your first season then well...you're bad. It's not an insult, it's a simple statement of facts. But you can improve. staying bad is completely up to you. pick a champion or two in every role and practice the everloving shit out of them. Go into draft pick and get as much practice in as you can during the off season/pre-season.If you want to climb then you are going to either have to practice or make a smurf.
The reason I say make a smurf is because your mmr is dead. You're going to have to play well above bronze if you want to climb. I lived in b5 for a long time and my mmr never dropped below 600 according to op.gg. you're in the low 400s. I can't even imagine what kind of games happen down there.
Now for the most important reason. You haven't said much so this could be wrong but the title gives me a pretty good clue. You tilt and you do it pretty hard. No one can help you here. You gotta fix that on your own. or get so amazing that you can solo carry every game. And to put that into prospective most golds and plats can't do it even against bronze and silver players.

you die way too much, i'm guessing you stay in lane past when you should and end up getting ganked? (ex. you get a kill and are at half hp and attack turret instead of B when the minions reach it)
If we still had carry.gg :'( I'd go through some specifics for you but instead let's talk about Bronze.
Bronze games are full of players who are missing some crucial piece of the League puzzle. Oftentimes this is CSing, warding, overaggression, an inability to group/take objectives(oh wait, that's silver too :O). I recommend playing more normals because your MMR there is probably a bit higher and more variable whereas it's hard to gain skill playing against people with the same or less skill than you. So you can work your way up in normals a bit and then when you go back to ranked your skill difference to others will be apparent(for me even a B2-3 skill level carried me up pretty noticeably) I recommend the youtube series leaguecraft 101. It's a little old and some of the specific champions he mention aren't as relevant but all the mechanics he talks about are still very important and I know that series helped me tremendously in getting out of Bronze.
Also don't ever give up on a game and try to encourage your team to do the same. Throws are so real in Bronze/silver it's funny. Keep looking for how you can come back in the game and help you team. Map awareness will also help you a lot in this because if you know where the enemies are you're less likely to die and could even get picks on targets out of position. Warding is important for this too, upgrade your yellow trinket as soon as it makes sense to (lvl 9-12), it doesn't matter what role you are (support with a sightstone then it's ok to go sweeper) just go for it and contribute to vision.
Don't forget after a fight to look for an objective and see if you team can take it. Just winning the fight and backing off is ok but if you can take a turret/dragon/baron/counter-jungle, something to get more income and deny the enemies you'll be so much better off.
Be willing to admit your mistakes and always try to learn, you have only room to improve from here so expect to gain something every game and you'll find value even in defeats.
Last of all remember that it's very possible for you to do it. I started out B5 last season with 0 LP and I was in much your position unable to really make it anywhere at first. I'm currently in S2 and I think I have a good chance of making Gold by the end of the season. Don't give up :)

Udyr is the only positive win rate you have and it's 61% for 23 which is above average.
Play more Udyr, duo queue with someone reliable. Try to keep the flaming down to a minimum.
I'm not gonna type a wall of text because I don't know your own personal mistakes. I see people offering all this advice, but it's so general and vague, it doesn't even pin point what's wrong for the OP.
But you seem to do well with Udyr, so play him for now, anything should be working in Bronze elo anyways.

Not enough champion kills.
I see you play alot with teleport. How often do you use it to help your teammates? I mean u are not that bad, your cs is above bronze level, your scores are fine, you are warding more than 90% of bronze.
I never saw you play, but what i think is possibility, you dont use your advantage to group with your team, instead you try to get more kills ,resulting in a throw. Also you should really not do off meta things in ranked like AP Thresh, Lightning Shen,
