im not a expert nor am i gold 1, but i can give you my common knowledge. basic mechanical skills like csing and making good decision aren't something that can be taught by someone, you learn it through practice, though it does help to try and replicate what pros are doing (as long as you know why they are doing it, and for what situation.)

Come by my stream sometime and I will be happy to spectate you and give you some tips based on what I see. (I will be on tomorrow but not tonight or Saturday.)

From your match history alone, one really major thing jumps out at me: you rarely buy boots. Boots are absolutely mandatory. On most champions, I would say that you need to buy level 1 boots around the time that you finish your first item, if not earlier. And you should upgrade them before finishing your second item. Having low movespeed gimps you in every way imaginable. You spend more time out of lane, it takes you longer to move around the map, it is harder to chase, harder to escape, and harder to dodge skillshots.

You do not need to "relearn" every lane to improve your mechanics...
