In tyrant Swain he is masked so you can't tell how he looks like.
In his current splash art he has a receding hairline and though he does look younger than his old self and receding hairline doesn't necessarily mean old , he still has dark circles under his eyes and wrinkles underneath it as well so he does look at least fifty for me.
The in game model is such a mess with the purple Mohawk that I don't know what to make of it...

The thing is that many people's perception of Swain is that he is much older. I think part of this is due to the cane and limp even though being a cripple doesn't necessarily imply age.
You would be surprised just how many meetings and discussions happen and will probably continue to happen about Swains age :)

seem to be the only examples of this, and many of them appear to be in the peak of their prime. It seems that this contrast would make Swain very distinct among the cast of LoL. Let's not forget that he manipulated his way into power with the help of the Black Rose - a plan that probably took years of planning and execution in order to be achieved. I doubt someone as young as 30-40 would've had the time and experience to conjure a coup like this.

He is a master tactician who has gained experience over the years but it's not clear if he's a genius who has a talent for tactics. However, based on his current lore and the coup that he orchestrated in order to consolidate power under his rule, he appears to be naturally talented. He has the charisma and ruthlessness to manipulate and control people who may not exactly agree with his vision of what Noxus should be (like Sion and the Du Couteau family). Based on his backstory and personality, he seems like he should be somewhere in his fifties at least. Reav3-senpai please?

Swain gives off the appearance that he is old, because his current iteration was terrible at portraying a withered, crippled middle-aged man. From his voice, to his outfit, to the way he grasps on to his stick for dear life when walking.
Keep the original concept, just improve on it and pull it off well this time around. Hell, you could even weave his lifesteal magic into his appearance, with the idea that the more he drains or the more health he has, the more youthful he looks.

He seems like a middle-age to older character. He's a focused tactician that's seen a lot of war and has been working for a long time to climb the power in Noxus. An injury can make you look older, but his raspy speech, darkened eyes, and gaunt appearance are signs of age. He's this wisened, harsh figure that's been tested by the world and is very powerful for it. He even becomes the ruler of Noxus. I think of a grand Wizard that's trained through pain most of his life and conquered a nation when I think of Swain. You couldn't make a character like that younger than 40 IMO.
Oakleaf Ranger2 votesKeep in mind the difficulties one has to face in growing up in Noxus if not born among the elites. The weak, dumb, and/or unlucky are unlikely to last long in Noxus. By the time he has reached adulthood, Swain's power and cunning would be years ahead of his age plus considering how talented/(hard working?) he would stand among best of his peers/age and potentially be in a league of his own compared to those of his age. Imagining him accomplish a feat like that in his 30's to 40's seems very possible when you think of it like that.

Most people think he is older because of his balding. About 25% of men begin balding at 30, and at 60 about 2/3 of men have gone through balding. Combined with the fact that he definitely has a somewhat wrinkly face he's definitely in at least his 40's.

I'm honestly not sure if that would be such a bad thing, though. His personality would have to very fleshed out for it to work, but it could work nonetheless.

Yorick honestly doesn't look any prettier than before. He just got a splash art that reflects the current skill of riot's artists. Compare both splash arts, both have an arnold chin and look grey/greenish, are hooded and pretty buff.
As for the models, same thing but alot more obvious.
The only one that failed to keep it tbh was Galio. He looks like a dumb musclehead and talks like macho man, how does that represent the old galio in any way?
While yes poppy failed on the personality/voice, her previous one was bland. Her looks are exactly the same, except the hammer/shield, just updated.

I mean new yoricks skin is a very light shade of grey/blue and I'm not sure how this is makes him more attractive than the previous one...

Vayne is in a skin tight suit, Ashe has a miniskirt, Caitlyn looks nothing like a sheriff, Kayle's an angel, Akali is practically nude, Anivia is a fragile ice phoenix, Irelia is very refined. Those champions don't seem very no-nonsense.
Poppy was grimy, no glamor, in your face deal with it, blacksmith who gets the job done. Now she's all adorable and bug eyed. I'm surprised you also didn't list Illaoi or Vi as for your examples.

All of those champions have very prim and proper personalities., as most of them are focused on their life roles as peacekeepers, nobility, etc. I didn't list champions like Illaoi and Vi because of that. They're significantly more fun-loving and spirited.
I'll admit that the "practical personality" is almost nonexistent in the face of wisecracking badasses, but Vayne hits that pretty much on the nose, and it suits her brooding huntress lifestyle and theme much better. Teemo also fits that niche pretty well for the same reasons. Keeping Poppy a no-nonsense yordle demacian would have way too much overlap with both these characters and prevent her from feeling unique. I feel that you aren't giving Poppy's current personality enough thought, as she's more "big-heart and big dreams" than "cutesy".
