He always had them.

You are new to the game? He always had the lips like that, and is not blood or lipstick. It's the flesh of the body he possessed, it's a dead person's body... I think he even mentions in his lore that he is walking with the stolen flesh of another man, becoming bigger and bigger the more he slays. I think the lips have that hue of a corpse :D I think is hot.
