The other two I'm iffy on. You might be right about Kayle (she'll likely get an update relatively soon, and they might make it more explicit then), but the Adaptive Help looks more metalic to me while Shen's covering looks more like cloth.

DrCyanide11 votesRaptor Cloak could probably be argued either way, but Xayah having a raptor skull on her cloak, Rakan's golden feathers near the edge, plus the coloring, makes it seem extremely similar.

You could argue it's a random chapter from a random book, but I think there's enough to connect it to Karthus specifically. First off, Karthus has a mana refunding spell in his kit, and the Lost Chapter provides a way to gain mana. This could be explained by coming from the same book that Karthus keeps with him, but since he uses his mana spell so often he has it memorized. Additionally, when Karthus pulls the tome out to use his ultimate he whips it out and pages start fluttering around. That seems like a great way for a chapter of a book to become lose and fall out.

Veigar is a maybe, but if so the hat has been heavily customized. Veigar's cap has spikes on it and a a large belt clip in the front, as well as plume from the top.
